NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED.
1. HOW TO ENTER THE SWEEPSTAKES
a. CBS Radio and Jones Apparel Group, Inc., �L.e.i. Jeans Sweepstakes� (�Sweepstakes�) will begin on 09/8/2008 at 12:00:01 AM Eastern Time (�ET�) and will end on 09/14/2008 at 11:59:59 PM ET (�First Entry Dates�) and begin again on 11/17/2008 at 12:00:01 AM Eastern Time (�ET�) and will end on 12/19/2008 at 11:59:59 PM ET (�Second Entry Dates�).
b. Visit participating CBS Radio station web sites during the Entry Dates and select the Sweepstakes page information/logo to open the Sweepstakes Micro Site window (�Micro Site�) or visit the Micro Site directly at www.leisweeps.com. To enter the Sweepstakes during the Entry Dates, following the online instructions. Entrants will be required to select their �Home Market,� and then submit their first and last names, complete address, city, state, ZIP code, telephone number, date of birth, and a valid email address in the online entry form. Failure to designate a home market will invalidate your entry. Only one (1) internet entry per person and per email address regardless of the home market designated. Internet entries will be deemed made by the authorized account holder of the email address submitted at the time of entry. The authorized account holder is the natural person who is assigned to the email address by an internet access provider, online service provider or other organization that is responsible by assigning the email address or the domain associated with the submitted email address. Multiple participants are not permitted to share the same email address. All entries submitted become the sole property of The Altitude Group (the �Administrator�) and will not be acknowledged or returned. Use of any device to automate entry is prohibited. Proof of submission of an entry shall not be deemed proof of receipt by Administrator. Administrator�s computer is the official time keeping device for the Sweepstakes. The Administrator is not responsible for entries not received due to difficulty accessing the internet, service outage or delays, computer difficulties or other technological glitches.
c. You may also enter the Sweepstakes by hand-printing the words �CBS Radio and Jones Apparel Group Inc., L.e.i. Jeans� Sweepstakes, your first and last names, complete address, city, state, Zip code, telephone number, date of birth and a valid e-mail address (if applicable) on a 3�x 5� card and mailing it in an envelope to: �Jones Apparel Group, Inc. L.e.i. Jeans Sweepstakes,� c/o CBS Radio, Attention K. Ehlers, 1515 Broadway 46th Floor, New York, NY 10036. The Administrator is not responsible for lost, stolen, mangled, misdelivered, postage due, illegible, incomplete or late entries
d. Only entries submitted during the Entry Dates will be included in the Sweepstakes random drawing. Entries submitted after the Entry Dates will not be eligible for the Sweepstakes drawing.
2. ELIGIBILITY RESTRICTIONS
a. The Sweepstakes is open to U.S. residents, age 18 or older who are residents of IL, WI, IN, TX, AZ, and FL. All employees of Jones Apparel Group, Inc. (�Sponsor�) and its affiliated companies, CBS Radio Inc., and each of their parent, subsidiary, and affiliated entities, advertising agencies, participating sponsors/promotional partners, any other radio stations respectively in the Chicago, Tampa, Houston and Phoenix, metropolitan areas and the members of their immediate families (mother, father, step-mother, step-father, siblings, children, spouse) or households are ineligible to participate or win.
b. Only one Winner per household is permitted in the Sweepstakes. Prizes will be awarded pending verification of eligibility.
c. Entrants are required to provide truthful information and the Administrator will reject and delete any entry that it discovers to be false or fraudulent. The Administrator will disqualify any entry from individuals who do not meet the eligibility requirements, and the Administrator will also delete any entry knowingly received from persons under the age of 13 in compliance with the Children�s Online Privacy Protection Act.
3. PRIZES
a. One-Hundred (100) Prizes will be awarded. Each Prize consists solely of one (1) $100.00 Wal-Mart Gift Card and one (1) of the following denominations $20.00, $10.00 or $5.00 f.y.e. Gift Card. Approximate Retail Value of each Prize: $105.00, $110.00 or $120.00. Aggregate value of all Prizes: $12,000.00. Wal-Mart Gift Cards and/or Online Gift Cards are not returnable or refundable for cash except in states where required by law. Resale of Wal-Mart Gift Cards is strictly prohibited. There is no expiration date or deadline for activating the cards. Once activated, the cards can be used in any Wal-Mart store or online at Wal-Mart.com. The f.y.e. Gift Cards are valid for purchases at fye.com and at any f.y.e. retail store. f.y.e. Gift Cards cannot be replaced if lost or stolen. f.y.e. Gift Cards cannot be returned or redeemed for cash.
b. There is no substitution, transfer, or cash equivalent for prizes, except that the Administrator may, at its sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the item(s) listed above and do not include taxes, gratuities or any other expenses. Other restrictions may apply. The Gift Cards are subject to the terms and conditions set forth by their providers.
4. SELECTION OF WINNERS
a. Decisions of Administrator with respect to the Sweepstakes are final.
b. On or about 09/15/2008 the Administrator will select a total of fifty (50) potential winners in separate random drawings from among all eligible entries received for each Home Market during the First Entry Period. Random drawings will be conducted for each Home Market, and winners will be selected as follows:
One (1) Prize will be awarded to each of the ten (10) winners selected from the Chicago market;
One (1) Prize will be awarded to each of the ten (10) winners selected from the Tampa market;
One (1) Prize will be awarded to each of the ten (10) winners selected from the Phoenix market;
One (1) Prize will be awarded to each of the twenty (20) winners selected from the Houston market.
c. On or about 12/19/2008 the Administrator will select a total of fifty (50) potential winners in separate random drawings from among all eligible entries received for each Home Market during the Second Entry Period. Random drawings will be conducted by market, and prizes will be distributed, one (1) Prize per winner, as follows:
One (1) Prize will be awarded to each of the ten (10) winners selected from the Chicago market;
One (1) Prize will be awarded to each of the ten (10) winners selected from the Tampa market;
One (1) Prize will be awarded to each of the ten (10) winners selected from the Phoenix market;
One (1) Prize will be awarded to each of the twenty (20) winners selected from the Houston market.
d. A total of one hundred prizes will be awarded.
e. Odds of winning a prize depend on the number of entries received in each Home Market during each Entry Period. The Winner need not be present or listening to win. Potential winners will be notified via email on or around the first day following each drawing as to how to claim the prize and will be required to execute and return a required Affidavit of Eligibility and/or Release of Publicity/Liability Prize Acceptance Form within ten (10) business days of winning or prize will be forfeited and an alternate Winner may be randomly selected. If a potential winner cannot be contacted, fails to sign and return the required affidavit of eligibility and liability/publicity release within the required time period, or if a prize or prize notification is returned as undeliverable, potential winner forfeits prize.
5. CONDITIONS
a. Payments of all federal, state and local taxes are solely the responsibility of the winners. Winners will be required to complete and submit an IRS Form W-9 with the winner�s full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by CBS Radio in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize.
b. Except where prohibited by law, by participating in the Sweepstakes, winners agree to allow their name, voice, or likeness used in any advertising or broadcasting material relating to this Sweepstakes without additional financial or other compensation.
c. Prior to awarding any prize or prize certificate, the Administrator in its sole discretion may require Sweepstakes winners (and their travel companions, if any) to sign a liability release, agreeing to hold the Sponsor, Administrator, and each of their parent, subsidiary and affiliated entities, and the officers, shareholders, directors, employees, agents and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the Sweepstakes.
d. The Administrator, in is sole discretion, reserves the right to disqualify any person tampering with the entry process, the operation of the Administrator�s website or Micro Site, or is otherwise in violation of the rules. Administrator further reserves the right to cancel, terminate or modify the Sweepstakes if it is not capable of completion as planned, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort.
e. The Administrator reserves the right to make changes in the rules of the Sweepstakes, including the substitution of a prize of equivalent value, which will become effective upon announcement. If due to circumstances beyond the control of the Administrator, any travel or event related to any prize is delayed, rescheduled, postponed or cancelled, the Administrator reserves the right, but not the obligation, to cancel or modify the Sweepstakes and shall not be required to award a substitute prize.
f. Failure to comply with the Sweepstakes rules may result in an entrant�s disqualification solely at the discretion of the Administrator.
g. The Administrator is not responsible for typographical or other errors in the printing, the offering or the administration of the Sweepstakes, or in the announcement of a prize.
h. For the names of the prize winners or copy of these Official Rules (please specify), send a self-addressed stamped envelope to: �Jones Apparel Group, Inc. L.e.i. Jeans Sweepstakes,� c/o CBS Radio, Attention K. Ehlers, 1515 Broadway 46th Floor, New York, NY 10036. Requests must be received by 12/31/08. The names of Prize Winners and the Official Rules will also be posted at www.leisweeps.com.
6. INTERNET SWEEPSTAKES PRIVACY POLICY
a. By entering the Sweepstakes, you agree to the Terms of Use Agreement and to the use of your personal information as described in the Privacy Policy located at: survey.cbsradio.com/vegas.
